I can see where your confusion lies. Let me try to elucidate.

Consider this PLB as spanning two realms: The "everyday" realm of Predominately Ordinary Organized People, and the "fantasy" realm of Creative Other-Worldly Dungeons. For our purposes here, let me refer to these henceforth as "POOP" and "COW Dung."

You seem to be approaching this with POOPy thinking. Try instead to think in terms of COW Dung. It is very POOPy to limit thinking to it being the SAME reincarnated monster 12 times. But to imagine that each adventurer is traveling on a separate-but-similar plane, facing identical monsters yet converging on a single final battle in the same plane, is total COW Dung. While it may seem that POOP and COW Dung can be used interchangeably, do not fall into this trap: POOP concepts must be "projected" when dealing with fantasy realms, so that COW Dung concepts can be firmly planted for the purposes of the battles. In other words, you must fling POOP, and step in COW Dung.
